Guam Cycling Federation

In consideration of and acceptance of this application, I hereby waive and discharge any and all claims for damage, death, personal injury, and/or property damage which I may have, or which may accrue to me as a result of my participation in the Guam Cycling Federation. The release is intended to discharge in advance the promoters, sponsors, Guam Cycling Federation, its Board of Directors, volunteers or its employees, against any and all liability arising out of or connected in any way with my participation in the event, even though liability may arise out of negligence or carelessness on the part of the person or entities mentioned above. I further understand that serious accidents occasionally occur during ride events, and the participants in these events occasionally sustain serious or mortal personal injuries and/or property damage as a consequence thereof. Knowing the risks of this event, nevertheless, I hereby agree to assume the risks and to release all of the persons or entities mentioned above whom through negligence or carelessness might otherwise be liable to me (or my heirs or assigns) for damages. It is further understood and agreed that this waiver, release and assumption of risks are to be binding on my heirs or assigns. I also permit my name, photo, video and interview to be used by the media, sponsors, and the promoters.
